CONSENT TO TAKE PART IN ANONYMOUS RESEARCH

TITLE OF STUDY: Human Decision-Making Experiments Using Internet-Based Software

PI: Tara Fahmie

This consent form is part of an informed consent process for a research study and it will provide information that will help you decide whether you want to take part in this study. For scientific reasons, this consent form does not include all of the information about the research question being tested. The researchers will give you more information when your participation in the study is over. It is your choice to take part or not. Your alternative to taking part in the research is not to take part in it.

Who's conducting the study and what is it about?

You are invited to take part in a research study that is being conducted by Tara Fahmie who is a faculty at the University of Nebraska Medical Center. The purpose of the research is to evaluate how people change between different responses based on whether they get rewarded for engaging in the different responses.

Tara Fahmie may be reached by email at tfahmie@unmc.edu or in writing at 6902 Pine St, Omaha Nebraska, 68106.

What will I be asked to do if I take part in the study?

You will be asked to click on the computer screen in order to get points. During the experiment, a colored background will be on the screen. You will need to use your cursor to click on the screen, and sometimes you will receive points. You will do this throughout the experiment.

The information will be anonymously collected. No one will know which responses are yours. Participation in the study will be between 10 and 60 minutes, which depends on the experimental group you are selected into. We anticipate that up to 1,000 subjects will take part in the study.

What are the risks of harm or discomforts I might experience if I take part in the study?

There are no significant risks related to your participation. The only risks and discomforts you might experience by taking part in this research include boredom and minor frustration from having to complete a mundane task.

Are there any benefits to me if I choose to take part in this study?

There are no direct benefits for you from taking part in this study.

Will I be paid to take part in this study?

You will be paid up to $10 per hour. You may be paid a bonus based on your performance during the experimental task (i.e., if you score more points, you may be paid more). The most you can get paid is up to $10 per hour for the experimental task.

How will information about me be kept private or confidential?

The research is anonymous. Your Amazon MTurk Worker ID or Prolific ID will be kept for the duration of the study to ensure that nobody completes the study more than once, but these data will be destroyed upon completion of the study. No other information will be collected that can identify who you are. As an additional safeguard, we will store all data on encrypted, password-protect computers and servers at the University of Nebraska Medical Center. Only researchers working on this experiment will have access to the data. Data on your performance in the study that is not personally identifiable will be kept for seven years following completion of the study.

What will happen to information I provide in the research after the study is over?

After the study is over the information collected for this research will not be used or distributed to investigators for other research.

The research team and the Institutional Review Board at the University of Nebraska Medical Center are the only parties that may see the data, except as may be required by law. If the findings of this research are professionally presented or published, only group results will be stated.

What will happen if I do not wish to take part in the study or I later decide not to stay in the study?

It is your choice whether you take part in the research. You may choose to take part, not to take part or you may change your mind and withdraw from the study at any time. If you do not want to enter the study or decide to stop taking part, your relationship with the study staff will not change, and you may do so without penalty and without loss of benefits to which you are otherwise entitled. Please note, however, that once you have submitted your responses, you may no longer withdraw them as we will not know which ones are yours.
